APPLICANT REQUESTS THAT:

His DD Form 214, Certificate of Release or Discharge from Active Duty, be amended to reflect he was awarded the following awards:

1.  Air Force Commendation Medal (AFCM), with two Bronze Oak Leaf Clusters (BOLC), instead of AFCM w/1BOLC.

2.  Korean Defense Service Medal (KDSM).  (Administratively resolved)



APPLICANT CONTENDS THAT:

He was awarded the AFCM (2BOLC) at the time of his retirement and he would like his DD Form 214 corrected to reflect this. 

The applicant’s complete submission, with attachments, is at Exhibit A.



STATEMENT OF FACTS:

The applicant initially entered the Regular Air Force on 8 Sep 75. 

On 30 Sep 94, the applicant retired under the Temporary Early Retirement Authority (TERA) after serving 17 years, 5 months, and 12 days of active service.  Block 13, Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized, of the applicant’s DD Form 214 reflects that he was awarded the AFCM w/1BOLC.

The remaining relevant facts pertaining to this application are described in the memorandum prepared by the Air Force office of primary responsibility (OPR), which is included at Exhibit C. 


AIR FORCE EVALUATION:

AFPC/DPSID recommends approval of the applicant’s request to amend his DD Form 214 reflecting the award of the AFCM (2BOLC).  A thorough review of the applicant’s submission and official military personnel record revealed documentation indicating that he was awarded the AFCM (2BOLC).  While his records did not include a Special Order, the applicant submitted what appears to be an authentic signed certificate and citation for award of the AFCM (2BOLC) for the award period of 4 May 93 to 30 Sep 94.  As for his request related to the KDSM, a review of his records also revealed that he served with the 51st Security Police Squadron, Osan Air Base, Korea, from 4 Jan 84 to 3 Jan 85 and is therefore eligible for the KDSM.  Therefore, his records will be corrected administratively.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:

A copy of the Air Force evaluation was forwarded to the applicant on 28 Jul 14 for review and comment within 30 days (Exhibit D).  As of this date, no response has been received by this office.



THE BOARD CONCLUDES THAT:

1.  The applicant has exhausted all remedies provided by existing law or regulations.

2.  The application was not timely filed; however, it is in the interest of justice to excuse the failure to timely file.

3.  Sufficient relevant evidence has been presented to demonstrate the existence of an error or injustice with respect to the applicant’s request for the Air Force Commendation Medal, Second Bronze Oak Leaf Cluster (AFCM (2BOLC)).  We took notice of the applicant's complete submission in judging the merits of the case and agree with the opinion and recommendation of the Air Force office of primary responsibility (OPR) and adopt its rationale as the basis for our conclusion the applicant has been the victim of an error or injustice.  Additionally, we note that AFPC/DPSID will administratively correct the applicant’s official military record to reflect the award of the Korean Defense Service Medal (KDSM).  Therefore, we recommend the applicant’s records be corrected as indicated below.  



THE BOARD RECOMMENDS THAT:

The pertinent military records of the Department of the Air Force relating to the APPLICANT be corrected to show that he was awarded the Air Force Commendation Medal, with two Bronze Oak Leaf Clusters (AFCM w/2BOLC), instead of the AFCM w/1BOLC.
